1 full mouth x-ray every 36 months
1 recall or specific examination, including polishing and cleaning, every 9 months
Oral Hygiene Instruction once every 9 months
10units scaling or root planning per calendar year (2 units for dependent children under age 13)
8 units occlusal equilibration per calendar year
Denture Relines/Rebases/Repairs once every 36 months 
Bridges and Crowns must be 5 years old to be replaced

Orthodontic Treatment:  
50% Payable

Orthodontic Maximum:  
$2,000 Lifetime
Must be under age 19 to claim Orthodontic benefit.
