Limits: 

• 1 complete examination every 24 months
• 1 full mouth x-ray every 24 months
• 1 recall or specific examination, including polishing and cleaning, every 6 months
• Oral Hygiene Instruction once every 6 months
• 8 units scaling or root planning per calendar year (2 units for dependent children under age 13)
• 8 units occlusal equilibration per calendar year
• Denture Relines/Rebases/Repairs once every 24 months

Orthodontic Treatment:  
50% Payable

Orthodontic Maximum:  
$1,000 Lifetime
Must be under age 19 to claim Orthodontic benefit.
