10 units scaling or root planning per calendar year (1 units for dependent children under age 13)
Denture Relines/Rebases/Repairs every 24 months
8 units for occlusal equilibration per calendar year. 
Bridges and Crowns must be 5 years old to be replaced
Polishing/ Flouride and Scaling, every 24 months
